Laser treats two of the four types of dark eye circle and neither of the other two. Pigmented circles respond to low-fluence Q-switched and picosecond 1064 nm lasers; vascular circles respond to the Vbeam pulsed dye laser; structural circles, which are shadows cast by a hollow or a bag, do not respond to any laser because there is no colour to remove. Most people who tell me that laser did nothing for their dark circles had a shadow treated as a stain. The four types of dark eye circle, and which respond to laser

The stretch test and the light test settle the type in under a minute at consultation. Darkness that persists on stretch is pigment or vessel; darkness that vanishes was a shadow. Pigment lasers for dark circles: Q-switched and pico 1064 nm
For brown pigmented circles I use a low-fluence 1064 nm Q-switched or picosecond Nd:YAG, after a course of topical lightening agents, because the topical step does part of the work and reduces the number of laser sessions. The 1064 nm wavelength passes through the epidermis with less melanin absorption at the surface than shorter wavelengths, which is why it is the pigment laser of choice in Fitzpatrick III to V skin. A trial of fractional Q-switched 1064 nm over six sessions at two-week intervals reported reductions in melanin index and improvement in lightness measured to three months after the last session. Sessions are spaced two to four weeks apart and the result is judged over months, not weeks, because periorbital pigment clears slowly.
Vascular laser for dark circles: Vbeam pulsed dye
Where the tinge is blue or purple and the vessels are visible through thin skin, the target is haemoglobin, not melanin, and a pigment laser makes no difference. The Vbeam pulsed dye laser heats the small vessels selectively. Temporary purpura for a few days is possible and is discussed before the first session. Where allergic rhinitis is driving the venous congestion, the laser is combined with treatment of the allergy, because vessels that are re-congested every night reappear as soon as the sessions stop; the allergies post explains that pathway.
Why lasers do nothing for shadow-type dark circles
A tear trough hollow or a Grade 1 eye bag casts a shadow that reads as darkness. Lasering the skin over a shadow lightens nothing, because the skin was never dark. A hollow is treated with tear trough filler and a bag with AGNES and Secret RF radiofrequency or surgery; the circle disappears when the contour that cast it is corrected. This is the single most common reason a patient arrives having had a course of laser elsewhere with no change.
Fractional laser for under-eye texture
Non-ablative fractional 1550 nm and 1927 nm lasers improve fine crepiness and surface tone under the eye. They do not lighten dermal pigment or close vessels, so they are an adjunct for skin quality rather than a dark circle treatment, used where thin, wrinkled skin is part of what the patient sees.
Laser for dark circles in Asian skin: the risk to manage
Melanin in the epidermis competes with the target for laser energy in Fitzpatrick III to V skin, and the result of an aggressive setting is post-inflammatory hyperpigmentation: a treatment for pigment that produces more pigment. I use low-fluence 1064 nm settings in preference to higher energy, non-ablative fractional in preference to ablative, sequential lower-energy sessions rather than one strong one, test spots where I am uncertain, strict photoprotection throughout, and I do not treat a patient who is currently tanned.

The number of sessions is set by type and by response, not by package. A pigmented circle typically needs a course rather than a single session; a vascular circle is reviewed after each session before the next is booked.

Does laser treatment work for dark eye circles?
For pigmented and vascular circles, yes, with the evidence rating lasers as mildly to moderately beneficial. For structural circles caused by a hollow or a bag, no laser helps, because the darkness is a shadow.

Can laser remove dark circles permanently?
Pigment that is cleared can return with sun exposure and rubbing, and vessels can re-congest if an allergy is untreated, so maintenance and cause control matter. Structural circles corrected by filler or fat treatment last as long as the contour correction does.
