In my consultations, the question about permanence usually comes straight after the question about price. That is understandable. Patients want to know whether they are paying for a lasting change or a short-lived improvement. Unfortunately, the answers they find online are often too simple. Either the result is described as permanent, or every non-surgical treatment is dismissed as temporary. The reality sits between those two positions.
I have performed AGNES and Secret RF eye bag treatments for more than 10 years and have followed over 500 patients. What I tell them is straightforward. The fat treated during the procedure is genuinely reduced; it does not simply grow back. But treatment does not freeze the face at its current age. The tissues around the eye continue to change, and that distinction is the key to understanding how long the result lasts.
What happens to the treated fat
A true eye bag forms when the lower eyelid fat pads protrude through a weakened orbital septum. In a mild Grade 1 to 2 case, AGNES delivers radiofrequency energy through micro-insulated needles into the protruding fat compartment. The targeted fat cells break down and are cleared by the body over the following weeks. Secret RF is then used to support collagen remodelling in the skin above, helping it settle over the flatter contour.
Once those fat cells have been destroyed and cleared, they are gone. Adult fat cells in a localised compartment do not replace themselves in the way skin cells do. So, if we are talking only about the fat that was treated, the reduction is permanent. However, patients rarely ask about just that. They want to know whether the area will always look exactly as it does after treatment. That is a different question.
Ageing does not stop after treatment
The orbital septum may have weakened because of age, heredity or both. Reducing the fat that has already moved forward does not stop the septum from changing. Years later, remaining or deeper fat may begin to protrude. The lower lid also loses support as soft tissues descend and the underlying bone changes. When a bulge develops again, it is usually not the old eye bag growing back. It is the ageing process creating a new one.
This is why I usually describe the result as lasting three to five years, although many patients do well for longer. That estimate comes from our own follow-up, not a marketing brochure. Younger patients with firm skin often remain pleased with the result well beyond five years. Someone with a strong hereditary tendency may notice change sooner.
Skin may become the main issue later
Removing a bulge is only part of the aesthetic result. The skin must also sit smoothly over the new contour. A patient treated at 38 may still have firm lower eyelid skin, yet by 48 the problem may be loose, crepey skin rather than protruding fat. AGNES was not designed to remove excess skin. At that stage, a procedure such as a pinch blepharoplasty may be more appropriate. The original fat reduction can still be present even though the patient now needs help with a different problem in the same area.
Not every recurrence is caused by fat
The under-eye area changes from day to day. Poor sleep, allergies, alcohol, salt, fluid retention and weight fluctuation can all make it look puffy. I occasionally see patients who are certain that their eye bags have returned, but examination shows that the fat pads remain flat. What they are seeing is fluid. That matters because treating fluid retention as though it were protruding fat will not help—and sometimes the useful intervention is as simple as reducing dietary salt.
No option stops the ageing process

Even surgery should not be sold as a way of stopping future change. It removes or repositions fat more directly, but the patient still ages and the lower eyelid skin may still loosen. The useful comparison is not “permanent” versus “temporary”. It is how each treatment works, how long its result usually lasts and what the patient may need later.
Who tends to keep the result longest
The pattern in our follow-up is fairly consistent. The best candidates have mild Grade 1 to 2 eye bags and firm lower eyelid skin. Treating the problem before the septum has weakened significantly also helps. I do not compromise on skin quality: if the skin is already loose, I recommend surgery instead. Stable weight and sensible daily habits help, but expectations matter just as much. Patients who understand the likely three-to-five-year timeframe are usually satisfied. Those who have been promised a result for life are being set up for disappointment.
What if the result changes later?
When a patient returns several years later, the next step depends on what I see. Often, no further treatment is needed: the change is minor, and the patient still looks better than before the original procedure. If a small new protrusion has developed and the skin remains firm, a maintenance AGNES and Secret RF session may be worthwhile. Subsequent sessions at The Clifford Clinic are priced at $1,800. If the eye bags have progressed to Grade 3 or 4, or loose skin has become the main problem, repeating a non-surgical treatment is unlikely to be good value. Scarless eye bag removal, with a pinch blepharoplasty where necessary, may then be the better answer. Looking after the result
Nothing stops ageing, but some habits make the lower eyelid age faster. Repeated eye rubbing stretches the thin skin and puts additional strain on the septum, so allergies should be treated and makeup removed gently. Daily sun protection helps because ultraviolet exposure accelerates skin laxity. Keeping weight reasonably stable also reduces changes in facial fat. Salt and alcohol can worsen fluid puffiness, which may obscure an otherwise good structural result. I also encourage patients to attend their scheduled reviews. Small changes are easier to manage while conservative treatment is still appropriate.
The honest answer
AGNES and Secret RF do more than disguise an eye bag: they reduce the protruding fat and improve the contour above it. The treated fat does not simply return. What I cannot honestly promise is that the patient will never again notice a problem beneath the eyes. The face continues to age, and skin that is firm at 40 may be loose at 55. Permanence therefore depends on what is being discussed. The reduction in the treated fat is lasting; the appearance of the whole under-eye area will continue to evolve. Any clinic answering this question without first examining the skin and grading the fat pads is giving a sales answer, not a medical one.
Frequently asked questions
Do eye bags come back after AGNES treatment? The fat treated by AGNES is cleared and does not simply regrow. A new protrusion can develop as the tissues continue to age. In our experience, results commonly last three to five years and often longer in younger patients with firm skin.
Is one session enough? For many mild Grade 1 to 2 cases, yes. A second session may be useful when the fat pad is larger, or the tissue response is incomplete. I make that decision at review rather than assuming every patient needs the same course.
Does surgery last longer? Usually, as surgery removes or repositions the fat directly and rarely needs to be repeated. It still does not stop ageing, and loose skin can develop later.
My eye bags look as though they have returned after two years. What should I do? Have the area examined before deciding what has happened. The puffiness may be fluid, a small new protrusion or increasing skin laxity. Each has a different treatment.
Can I keep repeating non-surgical treatment instead of having surgery? Not indefinitely. Once the eye bag has progressed to Grade 3 or 4, or the skin is loose, further non-surgical sessions usually offer diminishing returns. Surgery is then likely to be both more effective and better value.
Will the treated area age naturally? Yes. AGNES removes existing fat rather than adding material, so there is nothing to migrate or gradually dissolve. The area continues to age with the rest of the face.
Should I wait until the eye bags become worse? Usually not. Mild Grade 1 to 2 eye bags with firm skin are the cases in which non-surgical treatment tends to work best. Waiting may allow the problem to progress until surgery becomes the better option.
