CaHA treatment in Singapore with Dr Gerard Ee, undiluted for contour and diluted for skin quality
Treatments

I offer CaHA treatment in Singapore in three forms. Undiluted CaHA with lidocaine is used for cheek, chin and jawline contour. Diluted CaHA is used for skin tightening of the face, neck, décolletage, hands and arms. Diluted CaHA in sequence with HIFU is used for lower face and neck laxity. CaHA for contour is $1,400 per syringe and diluted CaHA is $1,800 per session, before GST. Performed by Dr Gerard Ee, MBBS (UK), MRCS (Edinburgh), PGDip Practical Dermatology (Cardiff).

Overview

CaHA treatment at a glance

What it does Undiluted, it lifts and contours the cheeks, chin and jawline. Diluted, it firms lax skin by stimulating new collagen and elastin.
Product Calcium hydroxylapatite (CaHA) microspheres in a gel, with lidocaine
Price $1,400 per syringe. Diluted CaHA $1,800 per session or $3,000 for 2 sessions, before GST.
Sessions Contour usually one session. Skin tightening usually 2 sessions, 4 to 8 weeks apart.
Time 30 to 45 minutes
Downtime Swelling and mild bruising for a few days
Results last 12 to 18 months on average

The evidence behind each use is set out in my CaHA filler in Singapore review.

Who suits CaHA treatment

A jawline that has lost its edge, or a short or receding chin, is given structure with undiluted CaHA placed close to the bone.

Flattened cheeks that need lift and projection, rather than soft volume, are treated with undiluted CaHA.

Thin, crepey skin on the neck, décolletage, hands or arms is firmed with diluted CaHA, which adds no volume.

Early jowls and a softening lower face are treated with HIFU first and diluted CaHA six weeks later.

CaHA jawline, cheek and chin treatment

Undiluted CaHA for cheek projection, chin proportions and jawline definition

Undiluted CaHA is placed deep, close to the bone, where the firm gel lifts the overlying tissue to define the jawline, lengthen or project the chin and restore cheek projection. The CaHA I use for contour contains lidocaine, a local anaesthetic, which makes the injection comfortable without separate numbing. Most patients need one to two syringes. The result is visible immediately and firms further over the following two to three months as new collagen forms around the microspheres.

Diluted CaHA skin tightening for the face, neck, décolletage, hands and arms

Diluted CaHA skin firming for the face, neck, décolletage, hands and arms

For lax or crepey skin, CaHA is diluted with saline and spread in a thin layer under the skin with a fine blunt cannula, which passes beneath the skin from a single entry point and lowers the risk of bruising. The diluted gel adds no volume. Skin firmness and thickness improve gradually over three months. I usually plan two sessions four to eight weeks apart. Use on the neck, décolletage, hands and arms is off-label, and I explain this before treatment.

What happens on the day of CaHA treatment

CaHA treatment appointment, assessment, photographs, treatment and review in 2 to 4 weeks

The consultation assesses skin thickness, facial structure and any previous filler, because CaHA cannot be dissolved and the plan has to be correct before the first injection. Photographs are taken and the treatment areas are marked. Numbing cream is applied for diluted treatment, whereas contour treatment relies on the lidocaine already in the product. Injection by needle or cannula takes thirty to forty-five minutes. A review follows at two to four weeks.

CaHA downtime and aftercare

CaHA treatment aftercare, avoid massage, pause exercise, saunas and facials for 48 hours

Swelling and mild bruising settle within a few days. Massage of the treated area should be avoided unless I advise it, because pressure can displace the gel before the surrounding tissue holds it in place, and strenuous exercise, saunas and facials are avoided for 48 hours. Because CaHA cannot be dissolved, I ask patients to contact me early if a lump or unusual pain appears, as an early nodule is simpler to treat than an established one.

CaHA with HIFU for lower face and neck laxity

HIFU first then diluted CaHA about six weeks later, with Tightan 2 and Sygmalift HIFU

For a softening jawline, early jowls or a loose neck, I combine HIFU with diluted CaHA, using Tightan 2 for a deeper lift or Sygmalift for gentler fractional HIFU. HIFU is performed first and diluted CaHA six weeks later, the sequence that produced the greatest gain in new elastin in a randomised study. HIFU tightens the deeper connective tissue layer, and CaHA placed six weeks later firms the skin above it while the repair response to the heat is still active.

Who CaHA treatment does not suit

CaHA treatment suitability, areas avoided and reversibility

CaHA is not used in the lips, around the eyes or between the brows, where thin, mobile skin raises the risk of visible lumps. It is not suitable during pregnancy or breastfeeding, over active skin infection, or for anyone who wants a filler that can be reversed. Patients with a history of keloid scarring or autoimmune disease need individual assessment, because both can alter the collagen response to the microspheres.

CaHA treatment questions answered

How much does CaHA cost in Singapore?

CaHA for contour is $1,400 per syringe. Diluted CaHA for skin tightening is $1,800 per session or $3,000 for 2 sessions. Prices are before GST.

How many syringes of CaHA do I need?

Most jawline, cheek or chin treatments use one to two syringes, depending on the length of jawline to be defined or the degree of chin projection needed. Diluted skin tightening is usually planned as 2 sessions, 4 to 8 weeks apart.

How long does CaHA treatment take?

CaHA treatment takes about 30 to 45 minutes, including numbing time for diluted treatment.

When will I see results?

Undiluted CaHA shows immediately, because the gel adds volume on the day. The firming from new collagen builds over two to three months.

Can CaHA be combined with HIFU on the same day?

Both can be performed on the same day, but I prefer HIFU first and diluted CaHA six weeks later, the sequence that gave the larger gain in elastin in a randomised study.

Is CaHA painful?

The CaHA used for contour contains lidocaine, and numbing cream is applied before diluted treatment. Most patients describe pressure rather than pain.
