Droopy eyelids are upper lids that hang lower than they once did, and most are not ptosis. Five causes account for them. Loose skin folding over the crease, a thick lid that weighs the fold down, a brow that has descended, a true excess of skin, and ptosis, in which the lifting muscle has weakened and the lid margin sits low over the pupil. The first three are treated without surgery at my clinic with AGNES and Secret RF, Thermage FLX Eyes and HIFU. The last two need a surgeon. This guide explains how the cause is identified, what each treatment does to the lid, and what treatment costs in Singapore.

What droopy eyelids are, the meaning of a drooping eyelid

An upper eyelid droops when something above the lid margin pushes it down or when something within the lid stops holding it up. Most patients who consult me about droopy eyelids are in their late thirties to sixties and have noticed the fold of the double eyelid becoming smaller, the lid looking heavy or hooded, or one eye appearing smaller than the other in photographs. A smaller group present with a lid margin that has descended over the pupil, which is ptosis. The two look similar in a mirror and are treated completely differently, so the first task at consultation is to distinguish them.
Causes of droopy eyelids

Loose eyelid skin
The skin around the double eyelid loses its recoil with age and sun exposure, because the collagen and elastin fibres in the dermis thin and lose their tension, and the loosened skin folds over the crease. The fold appears smaller, the lid looks tired, and eyeliner is hidden inside the fold. This is the most common cause seen at my clinic, and it responds to radiofrequency tightening because heat in the dermis contracts the remaining collagen and stimulates new collagen.
A thick, heavy lid
Some lids are heavy because the soft tissue between the skin and the tarsal plate, the firm plate that gives the lid its shape, is full. The weight of that tissue flattens the crease and adds to the sag. This is common in Asian eyelids and is often present from the twenties, since it is a feature of the structure of the lid rather than of ageing. It responds to Secret RF, which heats and reduces the bulk, usually with AGNES for the skin over it.
A brow that has dropped
The brow descends with age as the tissue that supports it loosens, and it pushes the lid skin down onto the lashes. The lid itself may be normal. A mild descent responds to HIFU, which tightens the support layer beneath the brow and lifts it by a millimetre or two. A brow that has dropped further needs a brow lift.
Excess skin, dermatochalasis
Dermatochalasis is a true fold of redundant skin that hangs over the lashes. No energy device removes skin, because tightening can shorten skin by a small fraction of its length but cannot take away a fold. The excess is removed by upper blepharoplasty.
Ptosis
The levator muscle that lifts the lid, or the thin tendon that attaches it to the lid, has stretched or weakened, and the lid margin sits low over the pupil. Ageing is the usual cause. Long contact lens wear, eye rubbing, previous eye surgery and, rarely, a nerve or muscle disorder are others, and each of them stretches or loosens that attachment over time. Some people are born with it. Ptosis needs surgery. AGNES with Secret RF, Thermage and HIFU do not work on it, because they tighten skin and the problem lies in the muscle. A droopy eyelid that appears suddenly, or that is accompanied by double vision, a headache or weakness of the face, is seen by a doctor the same day, because that combination can signal a nerve problem rather than an eyelid one.
Droopy eyelids vs ptosis, how to tell them apart

The lid margin, the edge that carries the lashes, is examined with the brow relaxed. Where the margin sits at or just below the top of the iris and the skin above it is what hangs down, the cause is loose skin, a thick lid or a low brow. Where the margin itself covers part of the pupil, or one eye is clearly lower than the other, the cause is ptosis. Raising the brows to see more clearly, or tilting the head back, also points to ptosis, because both are ways of compensating for a lid the muscle cannot lift. I confirm this at consultation by measuring the margin and lifting the brow by hand, which takes a minute.
Droopy eyelid treatment in Singapore, the options at my clinic

Three non-surgical treatments cover the first three causes. Each is described below, and the sagging eyelids treatment page compares them side by side with prices.
AGNES with Secret RF, the non-surgical eyelid lift
AGNES uses an insulated W needle passed over the dermis of the eyelid to heat the skin around the double eyelid, so that the collagen contracts and rebuilds and the lid lifts. The insulation confines the heat to the needle tip, which protects the skin surface and the eye beneath. Secret RF, delivered through an insulated cannula over the fold, heats and reduces the bulky tissue that weighs a heavy lid down. Together they work especially well on a heavy eyelid, the thick lid under loose skin, because each treats a different layer. The treatment is one session under numbing cream and a small local injection, takes about 20 minutes, causes two to four days of swelling, and allows a return to work from day two. Thermage FLX Eyes
Thermage FLX Eyes delivers monopolar radiofrequency through a 0.25 cm² eye tip, with a shield on the eye and nothing entering the skin. The tip heats the dermis in bulk while the surface is cooled, so that collagen contracts at once and new collagen forms over the following months. Thermage FLX is FDA-cleared for the non-invasive treatment of periorbital wrinkles including the upper and lower eyelids. In its 72-patient trial, 88 per cent of patients had upper eyelid tightening and 86 per cent had less hooding after one session, with no serious adverse events [1]. HIFU Eye
Focused ultrasound placed under the brow and along the outer eye heats small points in the support layer beneath the skin, which contracts as it heals and lifts the brow by a millimetre or two, opening a hooded lid from above. It complements the eyelid treatments rather than replacing them, and it can be performed with Thermage at the same visit. Droopy eyelid exercises, creams and home devices
Exercises work the muscles, and a droopy eyelid caused by loose skin is a collagen problem rather than a muscle one. Creams hydrate the surface and do not reach the dermis where the collagen sits. Tapes and glues lift for hours, and the repeated pulling stretches the thin lid skin further over time. Home tightening devices are too weak to heat the dermis and are not designed for the eyelid. Sunscreen and not rubbing the eyes slow the loosening, because ultraviolet light and friction both break down collagen. None of these measures lifts a lid that has already dropped.
Surgery for droopy eyelids, when it is needed
Excess skin is removed by upper blepharoplasty. Ptosis is repaired by shortening the levator muscle or reattaching its tendon. A brow that has dropped a long way is repositioned by a brow lift. An eyelid that has never had a fold is given one by double eyelid surgery. All four are performed at The Clifford Surgery at the same address, and I refer there at the first consultation when the cause is surgical, rather than after a course of energy treatment that cannot correct it.
Droopy eyelids and eye bags, treating the upper and lower lid together
A droopy upper lid and a bag under the lower lid often present together and are planned together. The lower lid bag is fat pushing forward through a weakened septum, which AGNES reduces from inside without a cut. Both lids are treated at one visit under the same numbing and are quoted together. Droopy eyelid treatment cost in Singapore
The AGNES with Secret RF eyelid lift costs $2,500 a session before GST, one fee for both devices, with the consultation fee waived when treatment goes ahead and two reviews included. Thermage FLX Eyes and HIFU for the eye area are quoted at consultation. Upper blepharoplasty and ptosis surgery at The Clifford Surgery are quoted by the surgeon. An eyelid trial offered elsewhere at about $250 is usually a partial treatment on a single device.

How do you fix droopy eyelids?
The cause is identified first. Loose skin and a thick lid are tightened and reduced with AGNES and Secret RF in one session. A mildly dropped brow is lifted with HIFU. Excess skin and ptosis are corrected by surgery. A treatment that does not match the cause produces no result.
Can droopy eyelids be fixed without surgery?

No treatment gives a lasting lift on the day. Radiofrequency shows a small change immediately, from collagen contracting under heat, and the full result at two to three months, once new collagen has formed. Tapes lift for hours and stretch the skin over time.
Do droopy eyelid exercises work?
Droopy eyelid exercises do not work. They work the muscles, and a droopy eyelid caused by loose skin is a collagen problem. They do nothing for ptosis either, because a stretched levator tendon does not shorten with exercise.
Can one droopy eyelid be treated on its own?
One droopy eyelid can be treated on its own, because the treatment is planned lid by lid. A single lid that has dropped suddenly or covers the pupil is ptosis until proven otherwise and is examined first.
Is droopy eyelid treatment painful?
AGNES and Secret RF are performed under numbing cream and a small local injection, and are felt as warm pressure for about 20 minutes. Thermage FLX Eyes needs no anaesthetic. HIFU is performed with numbing cream.
How long do the results last?
AGNES with Secret RF lasts about a year, sometimes longer, with a second session considered at the eight-week review for a thick lid. Thermage lasts a year or two. HIFU lasts about a year.
Is there downtime?
AGNES and Secret RF cause two to four days of swelling, with a return to work from day two. Thermage causes little to no downtime. HIFU causes none.
