Aching, stiffness and soreness in the neck and shoulders are hardly a rare occurrence in modern adults. In fact, up to 7 in 10 adults had experienced neck and shoulder pain some time in their life. Sometimes, the muscles can be so tense that it appears enlarged, and visible “knots” appear along the shoulder line.

The use of Botox to relieve sore and stiff muscles is not a new treatment but is often overlooked as compared to Botox’s more well-known uses in aesthetics medicine. Below, I will explain how Botox is used on the trapezius muscles to address neck and shoulder pain, as well as to sculpt the appearance of the neck and shoulder line.

What is Botox?

Most people, even those who know little aesthetics medicine would likely have heard of Botox, and roughly know what it does.


“Botox” is the common trade name for the medically used form of Botulinum Toxin A, a paralysing agent derived from the Clostridium botulinum bacteria. Although it is marketed under multiple trade names like Dysport and Xeomin as well, Botox is by far the most well-known and widely used name.

In modern medicine, Botox has a wide range of uses because of its muscle relaxant properties, from the most well-known use to reduce facial wrinkles in aesthetics medicine as well as to relieve chronic muscle pain and contractions like cervical dystonia.

What is the trapezius muscle?

The trapezius muscles are pair of triangular shaped muscle groups that extend from the upper back to the lower neck and laterally to the shoulders. Its main functions include the extension of the neck and movement of the shoulders. Besides its functions, the trapezius also plays a big part in aesthetics. The superior (upper) half of the trapezius muscle is one of the most prominent features in a person, forming the neck line and shoulder line. Its appearance can have subtle, but noticeable effects on the frame and figure of an individual, making them look more masculine or feminine depending on its shape.

  • Use of Botox on the shoulder line

Botox works primarily by relaxing/weakening/partially paralysing the muscles they are injected into. Other than its uses in wrinkle reduction on the face, doctors also use Botox on other muscle groups for both aesthetic and functional purposes. In my practice, I also use Botox on the trapezius muscles to both relieve stiff muscles and also to sculpt the neckline.

  • Relief of neck/shoulder stiffness, aches and pain

Stiffness and chronic pain in the trapezius muscle is a very common complaint in modern society, especially since many people today have desk-bound jobs. This jobs often require one to sit still and look straight for long hours, which can easily make the shoulder and neck muscles tense. Stiff trapezius muscles can cause chronic neck, upper back and shoulder pain and aches, occasionally leading to persistent migraines. Sometimes, visible “knots” that are tender to the touch can be seen along the shoulder line. In this case, Botox’s muscle relaxant properties come into play. Careful injection of Botox into selected points along the shoulder line can effectively relax the tense trapezius muscles, relieving pain and discomfort

  • Sculpting the neck and shoulder line

A slim neck and shoulder line is one of the features that accentuates a feminine appearance. Conversely, a well-developed muscular neck line can make one look more masculine, and may make the neck appear shorter in some cases. For those who wish to reduce the apparent size of their trapezius muscles for a more feminine frame can have Botox injections to relax and weaken them. This slims down the neck and shoulder line and can make the face look smaller. Also, those with an asymmetrical neck and shoulder line can have Botox treatment on the more developed side, balancing the 2 sides out. Finally, visible “knots” caused by tense muscles can also be smoothened out. Overall, Botox injections at well-selected spots can re-sculpt one’s silhouette to be more feminine and attractive.

Results

Like most Botox treatments, many of my patients see slight results in just 1 to 2 weeks. The results will continually improve for the next 2 to 3 weeks, with the best effects showing in about a month. These results will usually remain for 6 to 12 months, depending on the individual.

However, if you have regular injections every 3 to 4 months for about 2 to 3 years, the Botox can permanently shrink your trapezius muscles, and you will only need minimal or even no treatments following that.

Some of my patients actually wish to address their neck and shoulder stiffness, or pain relief without altering their neckline. This is completely doable by varying the injection points and Botox units.

Final words

Stiff shoulders and necks are some of the most common chronic muscle problems in modern adults. Although a good massage can sooth the pain temporarily, it is definitely not a long-term solution. Botox for the trapezius muscles not only alleviates the pain from tense muscles, it can also slim down your neck and shoulder line when performed correctly.
