AGNES and Secret RF for the Upper Eyelid: Tightening Around the Double Eyelid and Reducing a Heavy Lid

Radiofrequency on the upper eyelid does two specific things in my practice. AGNES, using its W needle over the dermis of the upper eyelid, tightens the skin around the double eyelid and creates a lift, so lax skin that had begun to drape over the crease sits back where it belongs. Secret RF, delivered through an insulated cannula over the upper eyelid fold, reduces the bulk of a thick, heavy lid, the fullness of soft tissue that weighs the lid down and adds to the sag. Neither lifts a descended brow, removes a true fold of excess skin or corrects a lid margin that sits low over the pupil; those remain operations. Why the upper eyelid is not the lower eyelid

A lower eyelid bag is herniated fat pushing forward through a weakened septum, which is why heating the fat pad from inside reduces it. Upper eyelid heaviness has more than one cause, and they call for different answers: skin around the double eyelid that has lost its recoil and folds over the crease; a thick, full lid in which the soft tissue itself is bulky; a brow that has descended and pushes the lid down; a genuine excess of skin, called dermatochalasis; and ptosis, where the lid margin itself sits low. Only the first two are radiofrequency questions. The last three are surgical, and I say so at the first consultation rather than after a course of treatment.

AGNES RF for tightening the skin around the double eyelid

For the upper lid AGNES is used with its W needle, a fine multi-point tip run over the dermis of the eyelid skin rather than the single insulated needle placed into fat for eye bags. The energy heats the dermis around the double eyelid, the collagen there contracts, and new collagen is laid down over the following weeks. The effect is a tightening of the skin around the fold and a lift of the lid skin, which is most useful where mild laxity has started to drape over the crease and soften it. It does not create a fold in an eyelid that has never had one, and it does not replace double eyelid surgery for a patient who wants a new crease.

Secret RF for a bulky, heavy upper eyelid

Some lids are heavy not because the skin is loose but because the lid is thick: the soft tissue between the skin and the tarsal plate is full, and the weight of it flattens the crease and adds to the sag. Secret RF, used through the same insulated cannula as for eye bags but advanced over the upper eyelid fold, heats the bulky tissue and reduces it, so the lid becomes lighter and the fold shows again. It is planned more conservatively than the lower eyelid protocol, because the upper lid skin is the thinnest on the body and the eye sits directly beneath it.

What upper eyelid radiofrequency cannot do

• Brow descent: the fold is lifted by repositioning the brow, not by tightening the lid skin beneath it.
• Dermatochalasis: a true excess of upper eyelid skin is removed by upper blepharoplasty; no energy device removes redundant skin.
• Ptosis: a lid margin that sits low over the pupil is a levator problem and a surgical one; radiofrequency does not touch it.
• A new crease: an eyelid without a fold needs double eyelid surgery, not radiofrequency.

I assess the upper and lower lids as separate questions, and I am more conservative about the upper one. Where the cause is a descended brow, dermatochalasis or ptosis, a course of radiofrequency costs money and months without producing the result the patient came for, and I refer to The Clifford Surgery, at the same address, for upper blepharoplasty or the operation that addresses it.

What the evidence shows

The published device studies support tightening of the upper lid skin, and both are small. A 2025 study treated 14 Asian subjects with a single session of monopolar radiofrequency to the upper eyelid and lateral orbit; 73 per cent reported more than 50 per cent improvement in tightening at two months and 86 per cent at six months, with brow elevation of about 1.2 to 1.5 mm and no adverse events. A 2023 study of microneedle fractional radiofrequency in 11 Korean patients reported improvement in periorbital wrinkle grade and skin elasticity. The lift around the double eyelid with AGNES and the reduction of a bulky lid with Secret RF are observations from my own practice over more than a decade of using AGNES around the eye; no controlled trial has measured either, and I say that plainly rather than imply one exists.

The procedure, downtime and risks

Numbing cream is applied for about 30 minutes and followed by a small local anaesthetic injection, a protective shield is placed over the eye, and the treatment takes about 20 minutes. Swelling for two to four days, pinpoint marks at the needle sites for a few days and occasional bruising are expected, and most patients return to work from day two. Burns, prolonged pigment change and injury to the eye are avoided by the insulated needle and cannula design, the eye shield and conservative settings. One session is planned, with reviews at four weeks and again at eight weeks, and a second session is considered at review where the lid is thick or the laxity is more than mild.
